Coastline Elderly Services, Inc. offers a full spectrum of programs to enable older persons to live independently in comfort and dignity in their homes, and postpone or prevent the need for institutional care.
- Long Term Care Assessment and Nursing Screenings
A process to determine eligibility as specified by Executive Office of Elder Affairs and Division of Medical Assistance. A Functional Impairment Level assessment is used to measure the individual’s abilities {ADL and IADL}and the level of care needed in response to the needs of the elder. This may include Home Care Services, one or more alternative programs/ services listed below, or a Nursing Home Screening to ensure appropriate services are provided. - Home Care Program
Care Management / Geriatric Care Manager: A coordinated program of Home Care Services to elders in order to assist and maintain maximum independence in their home environment. The service begins by conducting a screening and comprehensive needs assessment for Long Term Care services, setting up services plans, coordinating and implementing services and managing them. It includes on-going re-evaluation of elder’s needs as well as monitoring the services being provided. A personalized care plan is developed with involvement from the individual/family and the interdisciplinary care management teams to ensure that service delivery is goal-oriented, appropriate and cost effective. - Home Health Services
Health services provided through a Certified Home Health Agency to Home Care clients who are determined to be in need of such services in order to remain at home. - Adult & Group Foster Care
The Foster Care Programs provide care and support in a home environment for individuals, due to medical, physical, cognitive or psychiatric problems. These individuals cannot live alone safely. - Nursing Home Screenings
Home Care R. N. determines the medical eligibility for clients to enter a Nursing Facility under Medicaid guideline payment. - Adult Day Health & Social Day Care
Daytime programs with supportive supervision, a hot meal at noon, therapeutic activities and socialization. Elders return to their homes at the end of the day. Programs typically run five days per week for six hours each day and transportation can be arranged. - 2176 Home and Community Based Waiver Services
Medicaid Eligible Applicants whom meet the Executive Office of Elder Affairs’ Home Care Program Services Eligibility Criteria, and the Division of Medical Assistance Nursing Facility admission criteria. With approval a service plan is developed based on client’s need. - Enhanced Community Options
This program is available through State Home Care Program. To be eligible, you must be in enrolled in Home Care Programs and meet program guidelines. Currently there are two enhanced programs: the “New” ECOP (Enhanced Community Options Program) and the Community Choices Initiative. - “New” ECOP have combined previous enhanced Home Care programs. Clients can be either Medicaid or non-Medicaid. Clients must meet clinical eligibility for nursing facility services and the cost of services must be two times greater than Home Care Program. Through the new ECOP clients are seen every three months by either a case manager or ASAP RN. In order to stay in this program, clients must meet the criteria annually.

- Community Choices Initiative
This program is designed to ensure that eligible MassHealth members who are enrolled in the Home and Community Based Waiver receive coverage for needed community services, for the purpose of delaying or preventing an imminent nursing facility admission. It is a joint initiative of the Executive Office of Elder Affairs and the Division of Medical Assistance. To be eligible clients must have Medicaid and must be enrolled in the Home and Community Based Waiver Program. They must also meet clinical eligibility for nursing facility services and be at imminent risk for nursing facility placement. They also must need one of the following:- a.Was actively seeking nursing facility care within the last six months;
b.Recently experienced a serious medical event, regression in physical or cognitive functional ability or a cumulative deterioration in functional ability;
c.Was discharged from a nursing facility within the last thirty days;
d.Is at risk of nursing facility admission due to the instability or lack of informal or formal supports
And has one or more of the following clinical characteristics:
e.Needs 24-hour supervision because of complex health condition;
f.Experiences a significant cognitive impairment;
g.Is unable to manage/administer prescribed medications;
h.Experiences frequent episodes of incontinence;
i.Requires daily supervision and assistance with multiple ADLs.Clients are tracked monthly and seen on a monthly basis by a Community Choices team member. -
Respite Care
Services may provide temporary relief for the caregiver in emergencies, planned circumstances, or from the daily stresses and demands of caring for the elder. The intent of Respite Care is to increase or maintain the capacity of an elder to remain in the community and to avoid the need institutionalization by strengthening his/her social and personal support system. -
Personal Emergency Response System
An electronic communication system that can summon emergency assistance 24 hours a day, and automatically dials for help. A remote button carried around the neck or elsewhere on the body activates the device.
