Coastline Elderly Services’ Priority Meal Program is honored to be the recipient of an Aging Achievement Award in the Nutrition category from The National Association of Area Agencies on Aging .
The award will be presented on Sunday July 17, 2011 at the annual n4a conference in Washington, DC.
Coastline Elderly Nutrition Priority Meals Program provides hot nutritional meals to HIV/AIDS consumers. It is funded under the Commonwealth of Massachusetts Department of Public Health.
The following services are also available to participants of the program:
- Nutrition Education
- Information of other services and referral to other agencies serving the HIV/AIDS community with written authorization from the consumer.
- Nutrition Supplement
HOW DO I GET PRIORITY MEALS?
A letter of support from your doctor is the first step in getting on the program.
You must then contact us directly at Coastline Elderly Services, Inc. 508-999-6400 ext.194 or ext. 188 or you may go through an agency that is already providing other services for you. They will call in a referral to us for meals. We accept referrals from:
- Doctors
- Hospitals
- Local VNA’s
- PACE
- Center for Health & Human Services
- N.B. Child & Family Services
- PAACA
- Catholic Social Services
- Healing Light Productions
- Movable Feast
- Immigrants Assistance Center
- Any Other Social Service Agency
